- UP 8-7 Form is not notarized. UP 8-7 Form
- Last date of activity not listed for the property on accounts greater than $50.
- Owner relationship not listed on multi-owner accounts.
- Property type code not listed on accounts.
- Interest rate not listed on interest bearing accounts.
- Beneficiary or owner not listed on matured or terminated life insurance policy.
- Submitting pre-escheated accounts that have not met the appropriate dormancy.
- Report is out of balance.
- Incorrect payee on check. Makes checks payable to the West Virginia State Treasury.
- Reports with no West Virginia owners. These reports are returned to the Holder.
